Timeline

I hv say this before buh no one give a good answer..i lost my own copy of the medical after gotten my visa..i just wanted to know if am good since the embassy has the copy of the Original copy

DS-3025 needs to be marked complete and the "apply for blanket waiver" needs to be checked. You do not need this for your K1 anymore (assuming you are approved and have the visa in hand). You do need this for your AOS (but if you cannot get it there are different ways to cure that too)

From my own experience, they won't even look at it at the PoE. However, you need it for your AOS(adjustment of status).

@Fr8dog pls if i can get that copy again from the med dept....what are the different ways to cure that too as mentioned?

If you cannot get the the copy;

- You can file the AOS and hope the form was correct and that it stays with file. As long as the medical was less than a year old and the vaccination record is complete they should not issue an RFE for this.

- You can have the vaccinations transcribed on a different form, by another approved doctor, and sent in the sealed record of that with the AOS. This costs money and you need to find a doctor that is willing to do that without charging you for another medical.
