help please..AP for travel.. DS-3025 or Medical exam I-693?

hi there , I am in usa on k1 visa ..and now I am adjusting my status...I have my medical exam in Pakistan on November 2015.

all I have now is a CD which I suppose is x-ray CD.

and a letter I think its Ds 3025, as it says at the bottom..with my picture on top left .

I also have a polio drops card from Pakistan which I did just before coming to usa.

I am sending my aos packet in next few days ...I am very much panicking if I have to fill the I-693 form or should I just send the Ds 3025 letter with polio card ???

do I have to send CD with packet also?

I DONT have much knowledge please answer yes or no...or new medical from US.

also I am confused about AP for travel document ...I have no intention to visist any soon and in the form it says I have to write date and number of days for my intented stay....what shall I do ...as I ll b travelling only if my parents or family sick or in emergency....do I need to write on sheet this thing....how it gonna work ,,,,do I have to erite a letter...to get AP which I want in case of emergency travelling?

I don't want any RFE's pls

pls help with simple and complete answer

i dont understand it ,,i read it bt stil dont get it ,,,,so whats the answer in simple words ...yes or no....is new medical or old medical is stilll valid???

i am not expert so want to know what i ll be doing ,,,pls

If the DS3025 says complete and / or waived then you don't need the I-693 but we have no idea what all you got. If the pending AOS takes longer than a year to adjudicate then you may get an RFE for it.

Put unknown in the part for the AP and no supplemental sheet is needed.

Waiting to skip ROC and get 10 yr GC due to over 2 year while pending AOS
AOS APPROVED Oct. 4, 2017 * Green card in hand Oct 13, 2017 !!!!!

First K1 denied after 16 month of AP. Refiled. We are a couple since 2009. Not a sprint but a matter of endurance.

SUMMARY OF IMMIGRANT VISA APPLICANT; US vaccination requirement complete(Requesting a blanket Wavier)

thia section has a ticket box.

what is Requesting a blanket Wavier? and if this medical is complte u think/

Don't bother with the I693 and if they want it they will RFE for it. If it says complete then it's complete.

It has only been ticked on polio vaccine ...not other vaccines listed in form ..yet it says complete at bottom ?

It is odd that there are no other vaccinations on your DS3025.

Maybe the vaccines weren't available, I don't know. That question should have been asked during the medical.

If I were you I would wait for an RFE and then go get what is requested.

I don't know if what you have is an exact copy of the DS3025 that was taken from you at POE and routed to meet up at AOS.

It is up to you to decide if you want to go to a civil surgeon and get it transcribed to a I-693.

Costs are about $200 for vaccination and much more for medicals, every place is different, so shop around.

You don't need a full medical, only the shots but many CS will tell you that you do need a new medical and it's up to you

to convince them by taking the K1 medical instruction sheet along with you to show them you do not need it.

You can search for the CS list here on VJ and on the USCIS CS locator.

Then you call first to ask for fees.

They take cash, no insurance.

I don't know what you mean by Dr. Registration.

If your petitioner is better versed in English he/she should read through the thread that was linked for you earlier about

medicals and vaccinations. I can not re type it all here.

Your DS-3025 is not filled out properly. You need to see a civil surgeon. The vaccinations that are required for AOS are listed in that pinned topic that was linked above in the first reply by Merrytooth.

Same condition as mine. I didn't get any vaccine in my home country so when I came here, I called a few CS and found one that agreed to only give me the required shots and sign I-693 w/o a full medical. I then sent the sealed I-693 with my AOS.

Could u please "name the vaccination" which u did ? I'll call CS on Monday ?

And did u actually fill another form or u filled i693 ...how did u send those vaccination record with you DS-3025 record ??

Tdap, MMR and varicella. If you already had chicken pox tell your CS and they will only give you the first two.

I was in the same boat as the OP as they only gave me Polio shot in Pakistan while the other 4 shots are the requirement for the AOS. I had the full medical done, coughed out 715 dollars. When I went for the AOS interview, they didnt even ask for my medical and when I told the interviewer I had the sealed envelope, she was surprised and said that you didn't need it on K1. I said it was in the letter you guys sent and its in instructions that I need those particular shots which were not given to me in Pakistan. Anyway she kept my envelope in the file she had on me.

So either she didn't know that I wasn't given the shots in Pakistan and I needed them now or maybe we don't. I suggest calling USCIS about it coz I certainly regret spending all that money for it.
