do we still need I-693 with DS-3025?

Hello everyone. I am in the process of filing AOS for my wife. She came with K-1 visa. She has DS-3025 form. I looked through it and noticed that at least one vaccine (Rubella) was not completed. Do we need to go to the doctor to get I-693 done, just to be in safe side, or do you think that DS-3025 is sufficient. It was issued on August 21, 2015. Thank you for your input.

The Rubella requires 2 shots; my K1 wife had both shots but her Vaccine Records showed the date of only the 2nd shot. We went to a Civil Surgeon, who said that I-693 requires 2 dates for Rubella, and it can simply be completed if we take another shot there. But my wife knew for sure that she had had both shots (she just didn't have records of the 1st shot). Surgeon then said that a simple blood test will satisfy the I-693; so we did that, then wait a week for results and the surgeon signs off the I-693.

So in your case, if you have records of only 1 (or none of) Rubella shot, then definitely you'll need to somehow take care of the 2nd (or both of) shot. You should google a "civil surgeon" in your area and visit him/her; these civil surgeons most likely have done numerous I-693's already, so will have the best advice for you.
