Hello,

Did a google search and did not come up with answers so starting a new thread instead.

Quick question on the vaccination documentation worksheet. Do I send a photocopy or do I send the original along with the i-864 packet?

The sealed envelope already contains form i-693 for when I took shots here in the US right? So I only need the vaccination documentation AND the sealed envelope and nothing else?

If you went to a civil surgeon and received an I-693, then all you need to include is the sealed envelope from the civil surgeon that contains your I-693.
