Hi everyone, i am about to file for AOS. Can some give me an example cover page and also, can my sponsor submit tax trancript for the past 3 years rather than W2s? How many passport photos will i need, also the marriage certificate already have a seal, do i have to the it certified again?

I think there are some examples of cover letters around here, just search for AOS cover letter and see what comes up. Be careful though as a lot of older posts may come up and the fees will have changed. You can also just re-do your K-1 cover letter and change the address to the Chicago Lockbox, state that you are applying for Adjustment of Status from a K-1 visa and then list what documents you have included and sign it.

Yes, you can use IRS transcripts. Only the most recent year needs to be submitted, but you can include the last 3 years if you believe it helps prove you will not become a public charge.

6 passport photos total, 2 for the I-485, 2 for the I-765 and 2 for the I-131.

You just need to include a copy of the marriage certificate you received from the court after your marriage was registered. That seal/signature of clerk on it means it is a certified copy you obtained from the court. No need to send the 'original' you received, and no such thing as getting it 'certified again'.
