SORRY GOT ONE MORE,

FOR THE IV PACKAGE, MY WIFE'S BIRTH CERTIFICATE DOES NOT HAVE HER MAIDEN LAST NAME ON IT BUT HER CURRENT MARRIED LAST NAME. IS THAT AN ISSUE?

THANKS A LOT.

A birth certificate is issued upon birth, as the name implies. How did her married name end up on there, if it is the original document?

NO IDEA SHE GOT IT TODAY FROM THE LOCAL AUTHORITY AND IT HAS HER MARRIED NAME ON IT. THANK YOU FOR YOUR ANSWER

Unless I am missing something, doesn't it basically imply that when she was born someone knew who she would eventually marry? To answer your question (assuming USCIS follows the same logic): Yes, this would be an issue.

Besides being supernatural :)

I GUESS WHEN SHE CHANGED HER PASSPORT TO MY LAST NAME IT CHANGED IN THE WHOLE SYSTEM AND IS ON ALL PAPERS. THATS WHAT IT SEEMS LIKE

Apparently, it is the norm in Bosnia that divorce or name change are entered onto a person's birth certificate, so I guess USCIS is aware of it. See below link for information about this:
