Hi, my fiancé is from the Philippines and I am having difficulties trying to figure out how to put her address on the forms, because she lives in a village and not just some city with a street number and name. Any help in pointing me in the right direction would be appreciated.

What you could actually do is ask her if she knows someone in her city that actually lives in a city that has street number. And better yet, they are close family members so they could receive any letters that you need.

Or the closest barangay, you can use that too.

Have you ever sent her a letter/post card/presents? You could put the same way you have sent something to her address before.

If don't, just ask her how she gets the mail and what address she uses. I think, if she had already got any kind of mail (bank statements, utility bills, a letter form a relative), you could use the same way her address is written on them.

Your fiancee's address is not vital, you could use a simple "general delivery" to the barangay post office in the province where she lives. Nothing is mailed to her of importance... so relax.
