I wasn't able to find much info on multiple filing waiver approvals and how to go about them, so I thought I'd relate my experience. I just got our NOA2, and the points I made in my waiver request were the following:

1) First petition was almost 17 years ago. Ex wife resides in US, is educated, law abiding resident who owns her own home.

2) Second petition was 4 years ago. Ex is law abiding, marriage was just disaster.

3) Third petition was denied because of my stupidity (kept submitting G325 instead of G325A) - this approved petition was for the same beneficiary.

In addition to stating my ex's (flame away, but remember the old adage about casting stones) were law abiding citizens not on public assistance, I also sent tax records and an employment confirmation letter which confirmed that if all affidavits of support were still in effect, I still had sufficient income.

I don't know what made the case. Maybe they don't bother for a third beneficiary after 17 years, especially when there is no criminal record. YMMV.
