Can someone provide a sample Form No.5 Advisory of Marriages (CEMAR)?

Hi, I read in this forum that we need a CENOMAR for the K1 interview, and if my fiance had an annulment, we would instead receive a CEMAR (Form No.5 Advisory of Marriages). The name of her ex-spouse would be there with an annotation saying that it was annulled.

I was just wondering if someone could post a sample of their CEMAR with that annotation wording, because I couldn't find anything on google. Obviously censor out the important information. I just want to know what to expect, and so we can go back to NSO if the CEMAR wasn't properly annotated. Thanks.

Read the 2nd paragraph of my post, I did say I wanted a specific one "I was just wondering if someone could post a sample of their CEMAR with that annotation wording". Is the Form 5 you posted acceptable at the K1 interview or does it really have to have an annotation? If so, I wanted people on VJ to have a sample annotated-Form5 to see. :)

My request is because of this comment:

We finally got our Form No. 5 Advisory on Marriages (a.k.a CEMAR). For those who are wondering what it would look like after a successful annulment, it says exactly this:

1. Date of Marriage:

Place of Marriage:

Name of Bride/Groom:

Remarks/Annotation: With Declaration Of Absolute Nullity Of Marriage

From what I've seen in other posts, if your Form 5 does not have that annotation after the annulment is finished (i.e. you've received your annotated marriage certificate and wait about a week after that), then you need to go back to NSO and show them your annotated MC so that they update your Form 5.

I am annulled and I requested for CENOMAR and that was what they gave me. I also have my certificate of marriage with the annotation on the side. So technically, when your annulled you have 2 docs to request from NSO: Certificate of Marriage and CENOMAR which has the info above. When I submitted the docs after my annulment they also updated my form 5. I hope this help.

Thanks Hank. I thought CEMAR is the Certificate of Marriage and CENOMAR is the Certificate of No Marriage.

They are. But the request form you complete at NSO is for a CENOMAR, then they issue whichever form fits your status.

Don't need a CEMAR for the petition application at USCIS, but will need it for the actual visa application at USEM .
