AOS Vaccination DS-2053 Query

I am gathering my documents to apply for my AOS, and I am not sure if i need to see a Civil Surgeon regarding my vaccination record.

I have the DS-3025 Vaccination Documentation Worksheet that the certified Immigration doctor in Brisbane gave me after my K-1 medical examination. It is marked "Vaccination History Incomplete" and "Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate. The column for 'Varicella' has 'Varicella Hx' written in the Completed Series column, and a tick in the column marked 'Contraindicated. It is also dated and signed by the doctor.

During the K-1 medical examination I was given Tdap, MMR and Influenza shots, and was given a certificate of immunization for this (Vaccination Documentation Worksheet). The doctor told me that I now had all required vaccinations to adjust my status in the US.

My question is: Do I need to submit a copy of this in the AOS application?

I have read contradicting posts on this site. Some say that you are still required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an envelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. However I have also seen a post whereby the person advised that if the Vaccination record is complete, then simply a photocopy will satisfy them, as they will already have a copy direct from the Immigration doctor, that would have been inside the sealed envelope that I surrendered to immigration upon entering the US.

So does this mean I will need to book an appointment with a Civil Surgeon in the US, and have the document signed again, and sealed to submit? Or can I simply submit a photocopy of the Vaccination record in with my application?

Any clarification on this would be much appreciated! As always in this process, there are those several questions that should be so simple, but stop you in your tracks and cause no end of grief! I must say that I am thankful for a speedy process to this point, and that I don't have to be apart from my now <3 husband! <3 during this part of the process.

^{We are in the same process, and we took the copy of the DS 3025 and took it the civil surgeon and he filled it and signed the I693 ( just one page) and hand to us in a sealed envelope.}

I have read that post on the link previously, and I think I should be good to just enclose a photocopy of the Vaccination record. I suppose if they do require anything further they will let me know with an RFE.

As much as I want to get it all right first time round, an RFE is not as upsetting at this stage, as I am now in the US, and can be with my husband for this part of the process.
