Hello everyone, I am asking this for my cousin. My cousin came here on K-1 visa last month, her medical exam was finished last April. She handed the sealed brown envelope to officer when she POE. She is preparing AOS package. We just want to make sure we are on right path because the K-1 AOS guide mentioned she needs to have I-693 filled by Civil Surgeon. But on the pinned FAQs post, it states we only need to submit DS-3025 with completed vaccination, instead of I-693. Could someone please verify her concern? Thank you so much.

Could you please have a look at her DS-3025 and tell me if it is sufficient? Sorry but it is really complicated to me. Thank you so much.

Varicella is marked not routinely available, which means they did not get that vaccination. The flu vaccination should have been marked as not flu season, because it is age appropriate. It is not flu season, so it is not needed though.

Varicella needs to be marked off as having been administered on the date given, or a VH for varicella history, if they had the chickenpox before. So, they need to find a civil surgeon that will do just the vaccinations and not insist on a new medical that is not required. They need to get the varicella shot, or have the civil surgeon mark off VH for having had the chickenpox already in the past.

On the supplement form (not attached), it is marked Varicella HAS DISEASE BEFORE so I don't know it will help?

Your DS-3025 is incomplete. There is no evidence noted on the form that you had varicella before. It doesn't matter if the supplement form will be accepted or not because you are also missing the influenza. Based on the date of your DS-3025, you are required to have this shot.

The varicella is marked wrong then. It should have VH in the 'completed series, write VH if vericella history' row. As it is filled out, it looks as though the varicella vaccination was not available, so you did not get one. You could try and send the supplement and highlight and point out the fact that you already had chickenpox and did not need the varicella, but the instructions state you need a properly filled out DS-3025. The best that could happen is that they accept it. The worst that could happen is that they send you an RFE and you would need to find a civil surgeon for the I-693 vaccinations transcription at that time. OR you can find one now and get the vaccinations transcribed onto an I-693 before filing so you have no delays in case of an RFE.

It is not currently flu season, so if you go to a civil surgeon now, it is not required or needed to get that now.

I think the person who filled the DS-3025 is checked the same for everyone in VietNam. My wife also got the same marks for both VH and flu as OP's friend. My wife had her medical exam on 12/18 which I believe that is in the flu season but they also put not appropriated age ( She is not younger than 6 months lol). However, she had flu shot one month later but it was marked on her Vietnamese Immunization Record instead on DS-3025 due to the fact she had interview before her flu shot.

Anyway, we decided to submit DS-3025 and her Vietnamese Immunization Record instead getting Civil Surgeon. The Civil Surgeon in my city kept convincing us to take medical exam, and if we dont take medical exam, then she will not do immunization by itself even we already explained and shown her the instruction of I-485.

My wife's I-485 is still at Initial Review for 3 months from filing date. We did not receive RFE yet, but who knows what will come later. If your friend can find Civil Surgeon that can only do immunization, then she should go for it to prevent any RFE that might have.
