i think would be find unless you don't report them what your disease, the main problem is TB and HIV

Reporting diseases is not optional! That is why you have the medical done by a doctor approved by the US consulate ...

The list of communicable diseases of public health significance is here http://www.cdc.gov/immigrantrefugeehealth/exams/diseases-vaccines-included.html - these are the ones that can be a problem.

But even if you have those, it seems that as long as you can get treated and clear of them (for the ones that can be cured obviously) and do a second medical you will be ok for a visa, assuming one has time to do all this before the fiscal year end. It doesn't appear to me that hepatitis B would exclude you - but I would suggest you call the panel doctor in your country to double check on that.

And incidentally, HIV is NOT an excludable disease! As odd as that may seem. It used to be, but they changed it a few years ago. The details are somewhere on the CDC website.

1. when should I and my spouse and son to have medical examination?

2. My petition just moved to new address, should I inform to KCC?

3. When should I submit form I134?

Medical exam - usually 1-3 weeks before the interview, depending on country. They have expiry dates (usually 6 months but can vary by country) so don't do it too early. Most doctors won't do the medical until you have the letter informing you of your interview date anyway.

I don't think you need to worry about informing KCC anything about the petitioner, submitting new address and I134 can be done at the interview along with civil documents.

Can I submit only I134 without supporting documents I.e. income statement or bank statement of petitioner? And how much income should petitioner have for guarantee my family ( me , my spouse and 4 years son)?

Google US poverty guidelines to see how much the petitioner will need to show he can support your family with. And no can't be submitted without supporting documents ...just signing a form is not proof he can support you...
