What Medical Exam and Interview Form(s) do I need?

Greetings!

Currently, the Petition for K1 visa has been sent to the US Embassy in Manila and has been reviewed. My Fiancee hasn't received the letter with information on what to do next, but we received notification from the embassy that they are ready to schedule the interview. So my question is, For the medical exam, as well as the interview, what do we all need? To my understanding they changed what one needs last year (October 2013) (source: http://manila.usembassy.gov/wwwh3023.html - Skim down to "Notice to K Visa Applicants" ).

Do we just need the DS-160 form for both the medical exam and the interview? Did they retire the forms "DS-156, DS-156K, DS-157, DS-230"?

Hi. Welcome to forum! Yes, DS-160 replaces all the other DS-forms.

Do check out the tips from these sites when doing the medical and interview at USEM. The most urgent step now is for your fiancee to pay the visa application fee, so she can go ahead with scheduling her interview appointment. She can work around with planning for her medical and acquiring for other requirements.after getting her interview date.

The DS-160 is the only form that needs to be complete by the applicant now for the interview, this form is not used for the medical at St. Luke's.

St. Luke's lists everything required by them on their website.

The embassy instructions letter (online) lists everything required by the embassy.

One last thing to be aware of is CFO which is solely a Philippines requirement but must be completed to exit the country.
