Timeline

As I understand it, on a K1 you only need a vaccination suppliment (the "I-693A") if you've had a medical exam with the past year (and this I have). However, I'm drawing a massive blank as to how to get this suppliment. Is there another form I'm overlooking? The only thing given back to me after my K1 interview and at the point of entry was my x-rays (which they never actually looked at), and the doctor at the med exam gave me an extra copy of my immunization records... help?

You need a Civil Surgeon in your city to transfer the info from the DS-3025 onto the I-693A. Make certain that you let them know that you do not need another physical! You only need the vaccination info transferred to the I-693A.

I don't believe I have that (the DS-3025)... :/

My husband called the dr. he went to in Morocco and they said that his family member could go pick up the vaccination record.....this is what is transferred to the I-693A....we received it by FAX today....Atif's AOS interview is Feb.1, 2007.....

Also, if you go to USCIS.gov and go to FORMS and Fees, you can get a copy of the I-693A and find a list of civil surgons in your state. Hope this helps!

this is the photocopy the dr. who did the medical should have given you. You take this to the civil surgeon and he has the I693A.

Timeline

You need vaccination records for MMR, Polio, Varicella, and Tetanus. They might also ask if you have had Hep A or B, but I don't think it is required. You should take the vaccination records to the Civil Surgeon. If you need one of the vaccinations, you can get it done at any clinic or even your town health dept and it will be cheaper. Just bring the records with you to the CS. You may have trouble finding a CS who will complete the vaccination paperwork without the full medical, but just keep phoning until you find one. Don't mention that you had the medical done out of country, this just makes them even more determined that they need to give you the full medical exam again. Just say that you had the medical done by another doctor and you just need the vaccination supplement. At least, that's what worked for me. It took calling about 10 differnet doctors until I found one who would do it.

My wife had her medical exam here in the US and the I-693 filled out by a CS on 11/11/05. Does she need to get the vaccination supplement since it's been over a year? Does she need a whole new exam?
