Filing for AOS

Im a little confused to what I need to include in the medical forms for AOS??

All I have is the closed D from the doctor in Haiti when he had his exam and shots done before our interview.

What else should I include in the AOS packet for medical? Does he need to get more shots?

So what you are looking at doing getting the DS-3025 that the doctor in Haiti gave you and getting that transcribed to an I-693. If he got all the shots that he needed while he was in Haiti then he will not need to get anymore shots. What I would suggest is calling a Civil Surgeon in your area, ask them how much it would be to get your medical records from the Haitian Consulate transcribed to a I-693. If you need any shots the CS will let you know when they do the transcription. I have the paper thats attached to the CD it says its the DS-3025 with the Doctors seal and stamp. Is that all I need or do I need to have the I-693 as well??
