We are filing out the DS260 and have a few questions we need answered. I sent this email to IV manila and NVC also to get some clarification. Does anybody on this web site really know the answer? I have searched this site for several hours and there is a lot of educated guesses but does anybody have the correct answers?

We want to get the DS260 filled out to get our interview appointment and to get our medical scheduled but don't want to fill out the online DS260 wrong.

1. Does it make a difference on log in if you log in as the principal applicant or petitioner?

2. On the personal information page we are we don't know what name to use maiden or married?? On her passport it list the maiden name but we are in process of changing that to the married name. We have an appointment with NSO and DFA and the passport will be changed to the married name before our interview. What name should we use in filling out the DS260?

3.The beneficiary passport will expire in one month we will have a new passport in the married name before our visa interview will it be acceptable to fill out the DS 260 with a passport that will expire soon?

4. On the medical vaccinations record issue. We don't have all the records of hers vaccinations nor do we know what she will need until after medical (because she is Pregnant this might be different) Before we have our visa interview we will have the medical examination and vaccinations completed. What do we put for the answer to this question about vaccination records?

I'm Filipino, but I'm filing from Japan cause this is where we currently live. Since I just filled on DS260 last September, I will give you the best answers I know... Which is based on my experience here on VJ and with talking to people... hope this helps...

1. Does it make a difference on log in if you log in as the principal applicant or petitioner? --- I don't know if it makes a difference. But, just to be careful... DS260 is supposed to be filled in by the principal applicant. Since it's the IV application of the few months back, only now it's all online. 2. On the personal information page we are we don't know what name to use maiden or married?? On her passport it list the maiden name but we are in process of changing that to the married name. We have an appointment with NSO and DFA and the passport will be changed to the married name before our interview. What name should we use in filling out the DS260? --- use her current name in her passport. They won't follow another name that isn't on her passport. Believe me I've tried. And from what I've read on here, if she gets her passport in her married name before the visa interview, she can have the visa written with that on it... Since her visa name (if approved) will be based on her passport.

What do we put for the answer to this question about vaccination records? --- what was the exact question? I forget. Just be truthful. If she doesn't have the vaccines yet, then she doesn't have the vaccines. The medical exam will make sure she gets the vaccines she can get prior to the interview anyway.

visa will be in passport name. You can use her married name on all the paperwork if she will be changing the passport before the interview.

Even if she's pregnant she still has to have all the vaccinations. She should make sure that she does not receive a live vaccine when she has the vaccinations done. Since you don't have them done now, you can put no and her vaccination proof will be in the medical records. Can she get blood tests done to see what she is immune to or did she never receive vaccinations?

Thanks for the information. We are preparing all the documentation we will need for the interview and any documentation that we have the remote possibility of needing. Here is what I did and am going to do prior to interview. Please comment or suggest any changes that anybody thinks!!

By the way Interview is scheduled for Nov 26th at 6:30 AM

We filed for my wife under the principal applicant. We filed for the step daughter under Petitioner she is only 6 years old and I am the stepfather

visa will be in passport name. You can use her married name on all the paperwork if she will be changing the passport before the interview.

This is complicated we filed the I-130 and all the original petition information under her married name. We do have many forms of identification under her new married name. The problem we have is here passport that is due to expire in 1 month is under here maiden name. We have intentions of changing this to the married name but can not until we get the NSO certified marriage certificate. We expected to have this already but we do not as of now. We do have the Local Civil Registry certified copy of the marriage certificate. I did email USCIS asking the question about the name and this is there response:

USCIS Questions and Answers

1. What is the Visa that we will be filing for IR-1, CR-1 or CR-2 ( my wife does have a daughter that is 6 years old)

2. On the paperwork that we are compiling and forms I am filling out do I use the new married surname or the old given name for my wife?

3. We will change many of her documents but some will be in the old name just will be impossible to have all the documents changed over by the time we file is this a big problem?

4. What will be an acceptable marriage documents?

If you are a US citizen residing permanently in the Philippines you may qualify for DCF for spousal petition (Form I-130) at the USCIS office in Manila, if not, you must mail your petition to our Lockbox in the United States. For appointment and filing information you may access our website at www.uscis.gov.

1. Immigrant visa will be under CR1 & CR2 category.

2. You may use the married name for your wife.

3. Other documents under her single name such as your wife’s birth certificate is acceptable.

4. Copy of marriage certificate either from LCR or NSO is acceptable.

This is what I did and plan to do:

Filed the DS-260 per instructions under her maiden name listed on the current passport

11-4-2013 she has an appointment to get new passport she can not get one under her married name until the NSO certified marriage certificate is finished ( should be done any day). She will re-new her existing passport under the maiden name and wait 7 days for it to arrive expedited. This way at the very least she has a renewed passport that is good for more that 6 months at the interview.

We scheduled another passport appointment for 11-16-2013 and hope to have the NSO certified Marriage certificate by then and we will get another passport under her new married name

We will take at least the 2 passports if we have we will take all 3 passports to the interview and explain that we were waiting for NSO records to get a new passport.

We are covered either way on this with the 2nd passport renewed under the maiden name or her 3rd passport under the new married name

I did just answered NO and in the comments section said That we would be current after the medical examination prior to the interview. Proudly Canadian Please explain the blood test!! We will not have the records most were destroyed in a flood. The only way we can get them is to go back to the doctors and get them but this is long and hard process because of being born in Provence. Both her and my stepdaughter have received vaccinations but not sure if they have al that is neededl. And my wife being pregnant is a concern for us.
