I have a copy of my DS 3025 or vaccination records and I got the shots last June 2013. Do I have to fill up the I-693 and see a civil surgeon?

Also, it has been 12 days since my marriage to the petitioner and my I-94 expires on September 25th. We can't afford the AOS until 2nd week of September. Is there a limit on when we can file for AOS? Is there a ground that I will be asked to leave the US in my current status as a married K1 Visa holder?

you just need to ask the civil surgeon to transcript the DS-3025 to I-693 =)

about AOS no problem as long that u guys got married within the 90days since the arrival of the beneficiary. goodluck!!!!

Thanks for the info. :) I'm glad I won't be deported or anything bc I failed to file AOS.

How much would a transcription be? Is there a list of qualified civil surgeons?

Please see this post about the DS-3025 and I-693. You may not need a civil surgeon at all > http://www.visajourney.com/forums/topic/428381-i-693-report-of-medical-examination-and-vaccination-report/?p=6206232
