St. Lukes will ask for the NVC letter that has the MNL case number. if you already have it, then your fiancee needs to bring it at St. Lukes.

She also needs to bring the appointment letter..

SLEC want a copy (or two) of the NVC letter you (the petitioner) received with her information and the MNL #.

If she got an email from the embassy confirming her appointment that should work just as well, make a couple copies of that. Everything else she will need is in this list, SLEC info is near the bottom:

Update: Please check the schedule of the counseling session before proceeding to CFO because it is country-specific and the slots are limited only to 15 per session (two sessions per day) for Manila and 12 per session (only one session a day) for Cebu. It is on a first-come-first-serve basis.

Required documents & "OTHER DOCUMENTS" they ask for at CFO: originals & copies of; CENOMAR (CEMAR and marriage certificate if married), certified birth certificate. Passport and copies of bio page of passport, visa page from passport, pictures together. From USC (copies of) - bio page of passport, birth certificate, divorce decree (if been married before) are the most common requests. The "other documents" clause can be ANYTHING with those nit-wits at CFO.

When you get to the 3rd floor, You'd be asked to fill up an Information Sheet. (Yes, you have to fill it up there, It's not the downloadable one on the CFO website.) Make sure you answer ALL questions/fields. Rendering this incomplete will be one of their criterias for you to be "HELD/ON HOLD" for one on one counseling.

Information Needed on the Form:

- Basic Information about you, the fiance(e)/spouse (Name, Address, Birthday, etc.; Your Birthplace is important too, specify which city/town/area/province)

- Basic Information about your petitioner (Sometimes you are going to be asked for the Birth Certificate copy)

- FULL MAIDEN NAME of fiance(e)/spouse's MOTHER

- Contact person in the Philippines, specifically immediate family as in Father, Mother, Siblings. (I forgot what happened to the other one who listed her cousin. I think she was asked for her cousin's birth certificate. Which got her on "Hold" status.)

- Landline Number of Petitioner. (I know, I know, most of you guys don't use landlines now. That's what was running through my mind too.) If no landline is available, Landline of Petitioner's Nearest Relative or Petitioner's Neighbor. (IKR!? Neighbor?)

- Petitioner's Educational Attainment, Degree/Course, Occupation

- Information on Petitioner's Divorce/Annulment/Death of previous wife or husband. You must have a copy of the Divorce Decree. (One lady was asked to show the Death Certificate of the petitioner's previous wife.)

- ZIPCODES. Yes, people were given a few minutes to find out their Zipcodes. Both the petitioner and the beneficiary. Never forget to write those.

** From the Family Code of the Philippines: it's 21-25 years old for advice and 18-21 years old for consent. (Some are posted on VJ that they were not asked for this, yet others have been asked, so be prepared)

She doesn't need one. She will be asked to produce it just to counter check if the MNL case number is correct. She should just say that she got the MNL case number through phone and had checked her status with it countless times so she is sure it is valid and correct.

based on my experience the following were asked from me:

• Valid passport
• Photocopy of passport biographic/data page (vertical position, short bp) 1 copy
• 3 pcs recent 2x2 visa photos Write ur name on the back and photos should be identical
• Appointment letter from the embassy (2 photocopies) - this is the "appointment confirmation" page after you scheduled the interview online
• NVC Letter (2 photocopies) if available - if none, she can still have her medical exam. just provide the case number u got over the phone, she will need it
• Online Registration form (2 copies)
• Fee: $223 (Paid in Peso at the prevailing exchange rate currently it is at P42 based on their site)

Note:

• For divorcee/annulled,do not forget to bring your annulment/divorce papers

If there is no NVC letter yet, Tell your fiancee to inform the clerk that you just got the MNL case number over the phone. And that you haven't receive the letter yet. And that she will just bring it at the embassy once you receive it. NVC will send us the letter if they already mailed our case to MNL

(i am saying this because this is what I told the clerk and it was fine with them and didn't ask me for more. though i brought my NOA2 just incase.lol. I had my medical exam a day after we got our case number over the phone. until now, we haven't receive any letters from NVC. our case just left at NVC today.. we got our MNL case number June 4th and the letter may arrive few days from now)

Also, tell your fiance to check the last page of her passport to see if St. Lukes stamped her PDS number. that is also our proof to the embassy that we had the medical exam.
