I-485 RFE Question - Medical Examination

Hello everyone!

After days of suspense we finally got our RFE letter in hand and to our relief it is pretty straight forward. The first part is to submit a completed and signed G-325A. The second part is what I have a few questions on. The RFE States:

"You are a K1 fiancee whose medical examination is unavailable. Submit a copy of your completed signed Department of State (DS) medical forms or submit a properly completed and signed Form I-693, Report of Medical Examination and Vaccination Record, for the applicant on Form I-485. The medical examination must be conducted by a civil surgeon who has been designated by USCIS."

My wife's Medical Examination was on 6/13/12 in Toronto Canada. I sent a copy of the completed DS-3025 with the I-485 application but apparently that was not good enough or it was misplaced. We never got a copy of the I-693 from the doctor. Can we go to our local civil surgeon and get the DS-3025 transcribed onto for I-693 or does she need a new physical. We can also possibly try to get a sealed copy of the I-693 from the doctor in Canada.

I'm pretty sure you can have a civil surgeon transcribe it for you, there will be a fee involved but it will be nothing compared to having to do the medical again, I think they usually charge around $20, try calling some of the surgeons from the list the USCIS provides and ask if they provide this service.

Your medical was lost so you need to do the whole thing over. They aren't just talking about the vaccinations, but all of it. They say you can include a complete copy of your foreign medical, but you don't have that.

It is annoying, but it happens.

Your medical was lost so you need to do the whole thing over. They aren't just talking about the vaccinations, but all of it. They say you can include a complete copy of your foreign medical, but you don't have that.

It is annoying, but it happens.

That's what I figured after reading it over a few more times. At least there is a place here that does it for $175. Thanks for the reply.

I'm gonna go to a Civil Surgeon, get them to give me the medical again and fill up my I-693.

We are going ahead and getting an I-693 medical exam today and playing it safe. I was thinking if you do have copies of all of your ds forms I wonder if they would have to be in a sealed envelope like the original copies you turned in at the border. Another $185 dollars down the drain. Oh well.

You are just sending back the 5 pages of the !-693, right!? No, DS-#### forms??? Right?

The Civil Surgeon will put the required papers in the envelope, and it is just the I-693. Hopefully the Civil Surgeon uses knowledge of vaccination practices in your country so you don't have to get new vaccines. You can also go get any shots you need probably for cheaper or for free from somewhere else; you have to return to the Civil Surgeon in a few days anyway to check the TB test.

Exam went great today. Super nice doctor we had. Yes we are sending the 5 pages of the I-693 form in a sealed envelope and no DS forms. We are heading back on Thursday and the doc will look at my wife's tb test and we will pick up the sealed envelope.
