what form is that? don't leave anything blank. I wrote the date with a pen and everything else was typed, it's was not an issue.

But it also states one can write None.

Which would be the appropriate correct thing to put in the "middle name" box if there is none?

Put N/A - it's less ambiguous.

I left my fiance's middle name box BLANK, i was not sure if to out none or n/a since NONE can be mistaken as a name.. can I go back with a pen and write n/a? or should I just leave it blank? also,,can i date it with a pen even if the rest is fully typed?

Yes, but the correct answer is "none" not "na" Why would they not mistake "na" as a name?

You can provide any combination of pen or printing as long as it is legible

Just write legibly N/A Do not leave anything blank It could cause an RFE and delay approval.

Do not put either and use dashes like "----" in that box.

You seem like a smart person Smiley,so I'm sure that your aware that not everyone is as fortunate as yourself and that there is a very real possibility of receiving an RFE if something is left blank on any USCIS form.
