What is I-693?

Hi,

Came to the USA 2 months ago on a k-1 visa and getting ready to file for AOS.On the k-1 flowchart,step 20 is about completing I-693?

Anybody have an idea of what it is?and the supporting documents if applicable.

1-693 is medical, vaccanations,go to uscis civil surgons locator on line,

Hi kori, when you had your medical exam in cameroon, the doctor gave you an enveloppe, with Xray and a vaccination supplement sheet. if you still have it take it to a doctor for a transcript, some people just send the paper sheet alone, when your medical exams is less than one year it's ok for k1 visa; you don't need to go to a doctor for the transcript.

If you were given a copy of the DS-3025 at your K-1 medical, then check it over and see if it is completed properly and that you have all the required vaccinations. If you do, then just send a copy of that with your AOS paperwork. If it is not, then you need to find a civil surgeon, get any missing shots and have your vaccinations transcribed onto an I-693.
