It asks for name & addresses of employers for the past 5 years. There is no way to fit the full names and addresses into the space given. Do they need full address or just city/state? Or do I need to submit that information on a separate piece of paper? If I use a separate piece of paper, is there a specific form I need to use to do so?

Nov 16, 2012 (Day 17) - Received Biometrics appointment info.

Dec 01, 2012 (Day 32) - Completed walk-in biometrics.

Dec 31, 2012 (Day 62) - Received interview appointment info.

Jan 19, 2012 (Day 81) - Received EAD/AP Card.

Jan 31, 2013 (Day 93) - Interview

Feb 04, 2013 (Day 97) - Received Approval Letter

Feb 05, 2013 (Day 98) - Received Green Card!

Well you didn't state what form it is. Because the G325A is address, city and state the DS-230 is just City and state.

You can attach a sheet of regular plain paper (the instructions tell you) you put the question it relates to and type or write it out. In the space on the form write see attach sheet.

Yes, try to type smaller, or abbreviated or use a separate sheet informing that sheet belongs to the specific question. I remember I had to abbreviate the address to make it fit. Dont worry, as long as the required information is included by you, ALL will be fine. They know people have to use separate sheets sometimes. :yes:

Sign that separate sheet and with the date you did it. Like dwheels76 well mentioned, In the space on the bio form you better write "see attached sheet". :thumbs:
