We recently got married and received the marriage certificate. Marriage certificate has the maiden name of the bride. Now we are applying for her AOS. I need to know the following.

1. Should we put maiden name on all the formst such as i864-i-765 and i485? Or we should put married name. Currently we do not have any document that shows married name. Married name is first name as Brides name, middle name is my first name and last name is my last name.

2. What is A number? Where do we find it? Is it same as Visa number?

3. There are 2 different numbers on the K-1 Visa? Which one is the Visa number?

4. on the form i-765, Who fills up the information such as “WHICH USCIS OFFICE”, “Date” and “Granted”

When filling out AOS paperwork, use the name you want to appear on your green card. Whether that's the married name or maiden name is up to you entirely.

The A number is your alien number that USCIS provided you. It is printed on the visa in your passport and also on your NOA2 approval notice. It starts with the letter A.

The visa number is the number in red, if I remember correctly. Someone else can answer that since I'm not 100% sure.

You fill it out, unless it states otherwise. I don't remember what the form looks like. Check out the example forms that VJ provides if you're unsure.

To add to the above, yes you can use the married last name, but I don't think you can change her middle name to your first name without a court order. Her name is say, Mary Ellen Jones and you are Jim Smith, and you want her married name to be Mary Jim Smith? The law is designed so that you can change last names with a marriage cert, but I don't think it works for middle names, especially your first name (maybe her maiden name for middle name would be acceptable).

There is nothing that says it is an "A" number either on the K1 Visa stamp or I-94. I guess I will fill up I-94 number then.

No. The I-94 number is NOT the A number. You can't substitute one for the other.

Find the A number on the visa in the passport or on the NOA2.
