For the packet 3 part of the K-1, the Fiancee Letter of intent is there any sample? Also does the Petitioner need to put what US Immigration address? thanks in advance!

Depending on which State that you live, your income is good to sponsor for 3 people including yourself. You can check the income qualification from this link,

As far as the letter of intent, you can use the address of the Embassy or the Consulate your fiance(e) will be having her interview.
