DS -230 – Checklist, Forms and Document Included

1. Checklist

2. Cover letter / Bar-coded cover sheet

3. Complete DS-230 Form Part 1 – signed and Part 2

4. Two passport photo

5. Notarized copy of Principal Applicant/Beneficiary's Intl. passport bio-data page

6. Notarized copy of Principal Applicant/Beneficiary's birth certificate

You'll need an original or a certified copy. I don't think a notarized copy will work.

7. Payment Receipt for Police certificate/certificate of good conduct for Principal Applicant/Beneficiary

8. Copy of Marriage Certificate

10. Copy of Certificate of Naturalization

You don't need to send this.

11. Copy of petitioner's International Passport

You don't need to send a copy of the petitioner's passport. The DS-230 package is purely for the beneficiary.

Additional documentation

12. Wedding program

13. Pictures of Principal Applicant and Petitioner

14. Skype records between us

15. Some email messages

16. My flight itinerary to Malaysia

17. Copy of school IDs

Items 12 to 17 do not go with the DS-230 package. DS-230 package basically consists of the bar-coded cover sheet plus the following documents for the beneficiary.

1. Biographic page of passport.

2. Birth certificate (original or certified).

3. Marriage certificate (original or certified).

4. Divorce decree if applicable.

5. Police clearance certificate.

6. Two passport-sized photographs with the beneficiary's name, date of birth, and case number written at the back.

This is what goes with the DS-230 package.

Please help  I need to mail tomorrow.

What more can I include or what should i take out - sending document to NVC?

DS -230 – Checklist, Forms and Document Included

1. Checklist- To prepare right documents for the IV package, you dont need to include in your IV package

2. Cover letter / Bar-coded cover sheet : Are two different documents. Bar-Coded Cover sheet is "Must" and Cover letter is "Optional"

3. Complete DS-230 Form Part 1 – signed and Part 2

4. Two passport photo : Write down beneficiary name and Case number on the back of Photos

5. Notarized copy of Principal Applicant/Beneficiary’s Intl. passport bio-data page

6. Notarized copy of Principal Applicant/Beneficiary’s birth certificate

7. Payment Receipt for Police certificate/certificate of good conduct for Principal Applicant/Beneficiary: I dont see any reason you have to put payment receipt for Police Certificate. Police Certificate is good enough but it has to be Original. But I am not sure how it works for your country

8. Copy of Marriage Certificate : It should be Original or Certified not Copy.

10. Copy of Certificate of Naturalization: If you upgrade your I -130 petition during USCIS, you have to send a copy of Naturalization with NOA2 to NVC to upgrade your Petition filling as LPR to USC. It is not necessary document for IV package.

11. Copy of petitioner's International Passport: What is International Passport mean?? By this time you should have or apply for US passport and it is not recquired document for IV package.

Additional documentation : I dont see any good reason that you have to add all these exta documents on your IV package. Your petition is already approved from USCIS....Save all these documents for Interview purpose....Hope other VJers will give better idea if I am wrong.....

12. Wedding program

13. Pictures of Principal Applicant and Petitioner

14. Skype records between us

15. Some email messages

16. My flight itinerary to Malaysia

17. Copy of school IDs

Please help  I need to mail tomorrow. Make sure your Invoice status Shows " PAID" before mailing your IV package

and you also have to include photocopy of each of listed documents

the biographic data page of the beneficiary's passport

Certified birth certificate of the beneficiary

Certified marriage certificate of the beneficiary and the petitioner

Original police certificate(s) of the beneficiary

good luck

6 and 8 - need on each one:

1. original and

2. photocopy

re: relationship evidence submitting with the DS-230. It's not required, but since yer husband is Nigerian living in Malaysia, I suggest you send it in regardless.

Sometimes my language usage seems confusing - please feel free to 'read it twice', just in case !

Hi Darnell, copy from http://travel.state.gov/visa/immigrants/info/info_3195.html: "Photocopy of Valid Passport Biographic Data Page". So I assume it is not needed to notarize the biographic data page. Am I correct?
