Is it ok if I submit photocopy of passport with my maiden name on there and not my married last name? Or should I just ignore it and not to include in documents? My passport is still valid until 2014. Wondering what should I do..

i had my passport cancelled and applied for a new one in my married name already . .

my wife (Indonesian) had a "page 4" endorsement with her married name....so her data page has maiden name....page 4 has married name (passport office made the note and applied a stamp)....someone in your country might be able to tell you if this is possible there and how to do it. that way you can continue using your passport and when getting a new one get the married name.

If you're referring to the IV package, then a copy of the bio-data page of the beneficiary's passport is REQUIRED. So, you have to include it.

And it's fine if it's in your maiden name. Mine was and still is. I never legally changed my last name to my husband's (before or now) and had no problems whatsoever in the process of getting my visa and green card.

We submitted the I130 in her married name.

+++ I130 name refected her married name

We submitted my wife's passport info to the NVC with her maiden name.

+++ I130 name changed to reflect her maiden name

We submitted my wife's new passport to St Lukes with her married name.

+++ I130 name changed to refect her married name

Note: The visa and INS paperwork will be changed to reflect the name of the passport submitted to St Lukes. As the passport they received

is delivered to the consulate,,, and if you pass the interview,,, the Visa is placed in that passport.

I little wordy,,, but, hopefully you understand
