Yep! Sent it in! It was marked and good.

The interviewer chap said that the vaccine form was "new this year."

Painless, but annoying, if you ask me.

did you include a copy of the DS-3025 with you AOS application?? if so was it marked as "may be eligible for blanket waiver"??

It clearly states in the I-485 and I-693 instructions that if the medical was done within a year only a vaccination supplement is needed, and you only need to see a civil surgeon for an I-693 vaccination transcription if your DS-3025 was not properly completed.

I am not sure why someone who had all the required vaccinations at the time their medical was done would be told at the interview to go to a civil surgeon and have them transcribed to an I-693. It doesn't make much sense, but we give USCIS what they ask for.

I would personally wait until I received an RFE for a transcription at the interview because it is more likely you would not need the I-693 and would save a hassle and money. Going without the I-693 is not a risk of denial or anything. IF an RFE was given for that, then it would not take that long to deal with the hassle and get it done, and the approval would not be delayed for long.

I have a question about the ds-3025.

Is that something that is given during the medical, or is it in the sealed envelope that you give to the Embassy during K1 interview?

.. We can't find ours anywhere, and are going to try to call the doctors tomorrow to see if the DR can give us another copy. I dont think we were ever given a copy of it during the medical. We are already approved, and POE is next week!

April 18th 2014 - I-485 in acceptance.

June 26th 2014 - EAD/AP approved.

July 2nd 2014 - EAD/AP card arrived in the mail.

August 2nd 2014 - Potential Interview Waiver letter.

Jan 22nd 2015 - Submitted service request to USCIS on delay.

Jan 28th 2015 - Answer to service request that it's still being processed, and apologized for the delay.

Feb 26th 2015 - Jan 26th 2015- Sent in EAD to be renewed.

March 4th 2015- Renewal of EAD received by USCIS.

April 8th 2015 - I-485 Approved / Welcome letter sent.

April 13th 2015 - Welcome letter recieved.

April 15th 2015 - Card mailed.

Some countries do not give a copy to the K-1 applicant. It would be included with the rest of the medical documentation that was turned in at your POE. If you are absolutely sure you had all the required vaccinations, you could add in a statement that you had them all done at the time of the K-1 medical and the DS-3025 should already be included with your file. The worst that could happen is that you get an RFE and then at that time you would need to see a civil surgeon for an I-693.
