Order a new one from the county in which you were born.

you can get one from National population commision in or go to your local Govt authority to get declaration of age

I consulted the General Hospital,was told its obtainable at the National Population Commission at Surulere in Lagos.

Babs Animashaun.Bode Thomas Bus Stop,Surulere National Population Commission NYSC Campus.No where is difficult to find in Lagos as long as u open YAH mouth .when u r at Surulere Just as for National Population Commission.Bike fare is N50.

Make sure u have your affidavit done already i.e Affidavit for Age declaration (It can be done there or in Ikeja for N500-N700)
