Timeline

Not sure if this is the right location to post this or not...but here's my question.

My fiancee arrived on K1 visa three weeks ago and we got married two weeks ago. We are not applying for her AOS and trying to fill out I693 (Page 1 and Page 5). We went to our family doctor and he filled out page 1 and page 5. Although, on page 5 (D3025) he checked the box that says "vaccine history complete for each vaccine".....so here's my question. My fiancee had her medical exam done in India and they also gave her a copy of the D3024 but that says "Vaccine History Incomplete" but also has "applicant may be eligible for blanket waiver because vaccinations not medically appropriate".

So which one should I submit with the 1693? The one that the doctor from the US signed or the one from her medical exam in India. I'm thinking USCIS already has the D3025 from India...so now I'm confused.

Any help would be appreciated.

the one that your doctor (he's in the civil surgeons list for your state i hope) filled out and signed. the US doctor is required because they're the ones who can see if the Indian vaccination process was completed or not and if there's a need for a completion, a new one etc. don't worry about the other form being incomplete. like you said by the time she got here, the additional vaccination required might not be required anymore because of her age. =)

Did he also complete the vaccination table and seal it in an envelope?? If so, you send that one. I alsso sent my DS-3025. Approved without RFE or interview in just under 3 months.
