I am in the process of helping my wife file out all the forms that need to be filled after our marriage. The forms are G-325a, I-485, I-765, I-131. The question I have is we just got married a few weeks ago and have not changed any documents of hers to my last name. So is it correct to fill out the form for here with my last name even though she has no documents of her own that show that? Also what should be included when sending everything in? Do I have to fill out a medical form or just send it something she might have had from before?

Second. If you legally changed her name on the marriage license, then her last name now is the same as yours and that's the one you need to use.

Whatever name she wants on her GC is the name to use on the forms. If she has taken your name and then she uses that and of course she wont have documents with her new name as yet if you just got married and they know that. But her new name is your last name and she needs to sign everything with that and use that for all forms.

She would have had a medical before she came here so she should have a vaccination supplement form DS 3025. If it says incomplete at the bottom then you will need to have it transcribed over to the I-693 take it to a CS and he fills in the vaccination part only ,Part 1.2 and 5 and they will sign it give it back to you in a sealed envelope and you send that in with AOS

You can find a CS in your area by going to USCIS site and type in your zip code and it will give you a list. You can only use these doctors associated with USCIS.

First, post your future AOS questions in the AOS forum.

Is the card 8x11? If so then that might be the DS-3025 form. It usually has the form number in small type at the bottom. Either way, you need to go to a civil surgeon and have the vaccinations transcribed to an I-693 form.
