As well, what do I need to send with all the paper work, NOA2 and marriage certificate I've heard, what else?

Thanks for the info, on the list of forms to fill out it does not have the I-693 form, but in the package checklist it says it is needed?? Is it needed since I just had a medical?

It's not on the list of forms to download because that is not a form that you fill out; rather, it is filled out by a Civil Surgeon. Whether you need one is a complicated question, but the short answer is that you need a vaccine transcription on that form, only if your vaccinations were not complete at your medical abroad. If there were any shots you were told to get, then you need to get them and get a vaccination transcription on I-693.

Ok so, I've had all my shots and have a record of them, so if I just send the records in with my application it should be fine?

No, you do not need to send records to them. If your vaccinations were complete at the time of your medical exam abroad, then there is nothing you need to do. Your complete vaccines will be reported on your medical report that you handed in at POE. If they were not complete at that time and you have since gotten them, then you need to go see a Civil Surgeon and have them transcribe the vaccinations onto form I-693.

There is a sticky about this subject at the top of this forum. Good luck.

Sweet, thanks for the info, so I don't have to send them anything from my medical abroad? I got a cd with my x rays as well as another envelop
