Also where is asks for hubby's A# if any (he doesn't have one) so do i leave blank or put None?

it doesn't say on the instructions about leaving blanks on these things that i can see... maybe i'm blind.. thanks

If "no" is the correct answer for the questions related to disabilities, then yeah, check "no". If you didn't check "yes" for any of them, then you should leave the follow up questions blank.

If your husband doesn't have an A#, then it's not applicable. So "N/A" would be the appropriate answer there.
