Hello Guys ,

Some DS-230 Questions

1. Q 14 . Name of spouse (Maiden or family name) : this will be her last name prior marriage. Right? What should I put for her Middle name? Should it be the one prior to marriage or post marriage?

2. Q 16 . Address of Spouse (If Different from your own) : My wife is the petitioner but she is living with me in India since last six months but her principal residence is in US so should I fill her US address or leave blank as if her and mine address is same in India ?

3. Q 24 . Mother's Family Name at Birth : What should mothers middle name be? Middle name before marriage or after marriage?

4. Q 32a/32b Person(s) name in 14 and 29 who will accompany you/follow you to the United States now : Person name in 14 is my wife the petitioner who is already a US Citizen.how should I anwser that question ?

(i would think) Her married name...

Yes, her maiden name...

List your wife's and kids (if any) name(s) as a person(s) who will accompany/follow you (later)...

Your repeated use of the term "middle name" when referencing maiden name is confusing. I have answered your questions in bold, however, I have done so assuming you meant maiden name and not middle name.

1. Q 14 . Name of spouse (Maiden or family name) : this will be her last name prior marriage. Right? What should I put for her Middle name? Should it be the one prior to marriage or post marriage? Does she have a middle name? Those typically do not change when people get married and the wife opts to take her husband's last name. If the wife did not have a middle name before getting married and opts to take her husband's last name, her maiden name becomes her middle name.

2. Q 16 . Address of Spouse (If Different from your own) : My wife is the petitioner but she is living with me in India since last six months but her principal residence is in US so should I fill her US address or leave blank as if her and mine address is same in India ? Don't leave it blank. I'm not sure which address should be listed here (others probably know better). However, I can say that if you are able to list the address that you live at together, the answer would be "we live together at my current address."

3. Q 24 . Mother's Family Name at Birth : What should mothers middle name be? Middle name before marriage or after marriage? See my answer for #1, it's the same

4. Q 32a/32b Person(s) name in 14 and 29 who will accompany you/follow you to the United States now : Person name in 14 is my wife the petitioner who is already a US Citizen.how should I anwser that question ? I'm not sure how to answer that given your specific situation. However, I believe the intent of that question is if your spouse was not a USC. Others can give you a better answer.

September 1, 2011: NVC case number assigned
September 2, 2011: AOS invoice received, OPTIN email for EP sent
September 7, 2011: Paid AOS bill (payment portal showed PAID on September 9, 2011)
September 8, 2011: OPTIN email accepted, GZO number assigned
September 10, 2011: Emailed AOS package
September 12, 2011: IV bill invoiced
September 13, 2011: Paid IV bill (payment portal showed PAID on September 14, 2011)
September 14, 2011: Emailed IV package
October 3, 2011: Emailed checklist response (checklist generated due to typo on Form DS-230)
October 6, 2011: Case complete at NVC
November 10, 2011: Interview - APPROVED!!!

Middle name is usually the name of father and after marriage women take their husband's given name as their middle name.

So in My case my wife's pre marriage name

last name first name middle name (her father name)

and post marriage name

last name (my last name ) first name middle name (my name)

EDIT : My wife is a US citizen and petitioner.

Middle name is usually the name of father and after marriage women take their husband's given name as their middle name.

So in My case my wife's pre marriage name

last name first name middle name (her father name)

and post marriage name

last name (my last name ) first name middle name (my name)

That's not always the case. My middle name was given to me at birth, and is not my maiden name. It's /part/ of my maiden name, but I still use my middle name as a part of my married name. IE: my initials at birth were JKB. My married initials are JKS. Same first name, same middle name, different last (family) name.
