I am a little confused. I had a medical at london, rec the DS-2053. It states results, vaccine history incomplete, applicant may be eligible for blanket waivers because vaccinations not medically appropriate. They said they do this for everyones forms as vaccines are never complete.

Does this mean I need to file the I-693, and pay to see a civil surgeon.

If possible I would like to hear from English people who have been through this process.

There are some vaccinations that may not be medically appropriate because you havent reached the certain age to have certain vaccines they may be required.. and i think there is a vaccine that uk doesnt even offer, therefore you are waived from having to have it at this time.

When and how is the fee paid to the department of state?

Once the K1 has arrived in the US they marry, then apply for AOS (Adjustment of Status) and the fee ($1070) is paid when AOS is filed.

I suggest you read the AOS guides (and the AOS thread in my signature).

If there are any shots that you are missing then you will need to see a CS and have a vaccination transcription done. If you don't think you're missing any shots then you can simply send a copy of the DS-3025. If you're not sure it's often easier to see a CS to make sure then risk an RFE.
