Cover letter for IV application packet

Hi - Does anyone know where I can find a cover letter for the IV application packet? I've already printed out the document cover sheet with bar code, but I saw a nice letter/cover sheet link somewhere on this site and can't find it now.

I dont know about others but recently when my wife went to get her Passport clearance she first had to get a police clearance from her local police stations... then attach that certificate when applying for passport clearance...

so when i sent the certificates in i had both of them attached... one was Passport Clearance and one was Police Clearance..

This is the first I've ever heard of passport clearance! It's not listed on the EZ spouse guide to which I've been refering.

It's only required in India, as far as I know. So you should be fine. You can always ask an NVC operator to verify.
