If you look at the guides here, or the instructions on USCIS, they both tell you the answer:

For I-130, you BOTH fill out the G-325A and attach the photos

For I-485, YOU (the alien) has to fill out G-325A

So - if you are filing both forms, you fill that same form for both of them (i.e. fill it out and print 2 copies..) and your spouse will fill out one copy, for I-130.

Just list the content of each application / petition in the cover letter. You can find plenty of examples from these forums.. I just posted mine here:

http://www.visajourney.com/forums/topic/299032-example-assembly-documentcover-letter-anyone/page__p__4525310__fromsearch__1#entry4525310

The fees for filing each form are listed on USCIS site and on this site.

for I-130, $420

for I-485, $1,070 (which includes the biometrics fee)

Attach each check to the corresponding form with a paper clip etc.

Send all in one envelope.

The ID and SSN are not required usually. I sent them because my husband's last name is misspelled on his birth certificate and the ID/SSN show the correct spelling, also my friend for an RFE for her SSN so I figured I'd sent that too just in case.

We sent two checks for $420 and $1070.

I had a brown envelope inside the priority mail envelope. The I-130 was clipped together with its relevant documents and the I-485/I-693/I-765 was clipped together with its relevant documents.

I clipped the whole thing together with the cover letter and G-1145 form on top of the whole thing.

I dont think it matters how you send it as long as its all arranged properly.

It's not necessary. I only sent one for the entire package. Some send an overall coversheet, a cover sheet for I-130 and cover sheet for I-485. Even if you don't send a cover sheet, they won't reject your package. It just makes it much easier for them. Myopia and Little_My both gave excellent examples of a cover sheet. You seem to be making this more difficult than you need to be. ;) Just relax!

If you and your wife are having problems - is she going to go along with sponsoring you for the AOS?

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

WARNING: Phrases in this post may sound meaner than they were intended to be. Read the Adjudicator's Field Manual from USCIS

I am guessing he meant to say they are having problem filling out the correct paper work or they were not aware of the correct paperwork to be done.
