Yes thats correct, family name means your familys last name. Your current last name.

Then any past names you used maiden or otherwise go in the other names used part.

VJ has a form example section for most of the forms you need to fill out. The blue bar at the top of the website where it says example forms, click it and then select whichever form you need to fill out from the list. Its got filled in helper info.

I-130 CR1 visa is what I would put there. For this purpose you sign as "applicant" on your G325a and your husband will sign as applicant on his G325a. When asked about his current wife, he'll provide your maiden name.

A person born as Mary Jane Smith but who married and took the name of Bob Jones is NOW Mary Jane JONES. In other names used, she would type Mary Jane SMITH, JONES for both the G325a and I-130 where asked. A touch doesn't mean anything at all. My advice is to spend your energy elsewhere as you may never see any evidence of a touch between filing and approval. If you do, you'll never know what it meant.

Some people attach too much importance to this "TOUCH" thingie. I'd say ignore it and concentrate on sending the right documents and hope for a timely NOA2(USCIS approval).

A touch is when they scan your files bar code, it shows as a last update on USCIS log on account information. It is a way for them to keep track of your files so they don't get lost. I wouldn't hold too much to it but I posted mine because the majority of my touches came when my AOS was approved. So if you make your USCIS account and don't see any touches don't get upset, or if you see a bunch of them around a certain time keep a watch out for a possible approval or RFE but don't get too excited.
