Does her vaccination record say complete? To be safe, have a civil surgeon transcribe her vaccine records into the i-693 to avoid any RFE's. She does not need a full medical. It should cost less than 50 dollars, thats what I plan to do!

My Journey:

We met through a study-abroad program in Shanghai, China in August of 2009

We got engaged March of 2010

I received my K1 VISA in 6 months (June-December 2010)

We were married 04/02/2011
I received my conditional 2-year greencard (AOS) in 2.5 months with no interview (April-June 2011)

Our son was born 02/03/2013

I received my masters degree in Speech-Language Pathology 04/17/2013

I received my 10-year greencard (ROC) in 3 months with no interview (March-June 2013)

My husband returned from deployment 06/20/2013

My naturalization journey took 4 months (April-August 2014)

I became a US citizen on 08/01/2014

Received passport in 3 weeks (regular processing)

The I-693 instructions say that an exam with a DS-3025 completed in the last (some period of time, maybe a year?) means you don't need the I-693... so we're taking our chances and sending the DS-3025 (which has the "incomplete" box checked, but also the "waiver please" box). The gummint already got an envelope full of results at the POE.

We do, however, have an I-693 form already printed out, with the first section completed, in case they want one. Just wanted to get the ball rolling on everything else while we have the money to do so (but not the money for the CS, 'til next paycheck or so).

I didn't want to take the chance either. We had the 3025 for both my wife and daughter. However, to be safe we got the Civil Surgeon to transcribe the data on the I-693 and seal it.

The only USCIS approved Civil Surgeon within 150 miles of us was glad to do it. Of course we didn't realize at the time that he was going to charge us $200 per person just to transcribe the data and put it in a sealed envelope, (15 mins). If you get the I-693 the civil surgeon must complete it and seal it with one of those "do not open" stamps.

I mentioned the excessive cost for the I-693 to our AOS interview person and she just gave us the "we have no control over what they charge", line.

Hello, thank you all for your help. My wife is here on a K-1. She has her DS-3025 vaccination history and completed her medical exam (in Bogota) within 1 year. Does she still need a US Civil surgeon to certify her vaccination history? Thanks,

Only if the DS-3025 was completed and a copy of it was placed in the POE envelope by the consulate will the USCIS use it as proof of vaccination.
