Am I missing anything?

Two things are wrong here, you need to submit the copy of Biographic page of the passport for the BENEFICIARY, not the petitioner (doesnt need to be notarized either). Also, you do NOT need to include a birth certificate for the petitioner. You already did that in the USCIS step. Remember to include one copy of everything you submit.

Dear Sir/Madam,

Hereby, please find DS-230 application and supporting documents for XXXXXXX, to acquire immigrant visa (CR-1).

Please note that, at this time we (are) unable to obtain PASSPORT CLEARANCE CERTIFICATE from the Ahmedabad Passport Office, India. According to the Passport Office, they would require either a "missing document checklist letter" or "Interview Appointment Letter" from the for verification purpose to issue the Passport Clearance Certificate.

So, for now we have attached a notarized copy of local police clearance certificate with this application (Original police clearance certificate is required to submit with the application for PASSPORT CLEARANCE CERTIFICATE).

Since we are unable to obtain Passport Clearance Certificate at this time from the Ahmedabad, Gujarat, India because of the above reasons, please provide us with the Checklist letter of missing documents, such as passport clearance certificate, or an Interview Letter, so we can obtain passport clearance certificate As Soon As Possible. Then and only then we can provide the PASSPORT CLEARANCE CERTIFICATE to the Consulate at the time of interview. Therefore, please complete our case and forward the case to the Consulate where we can submit the police clearance certificate.

Enclosed are the following:

1) Barcode sheet

2) Cover letter

3) Copy of the missing and incomplete checklist letter of the NVC

4) DS-230 package part I (signed) and II

5) 2 passport size photo of the beneficiary

6) Beneficiary Birth Certificate along with two affidavits and a copy

7) Marriage Certificate and a copy

8) Notarized copy of Local police clearance and a copy

9) Notarized biographic page of the beneficiary's passport and a copy

10) Copy of Biographic page of the Passport – Petitioner and a copy
