my fiancee just entered the usa a little over 2 weeks ago and we will be married in about another 2 weeks, my question is does she need anything from the civil surgeon to apply for her aos or does she not need any of this till she goes for her first green card? from looking at the paperwork for the aos i see nothing asking for anything from the civil surgeon on the inital paperwork.

I beleive form I-693 she will need to accompany the AOS paperwork which is her vaccines she maybe already obtained in her country she will need the civil surgeon to transfer her vaccines from her sheet to the I693 form.....

It depends on whether she's received all series of the required vaccinations before coming over or not, and she has the vaccination record/supplement to include in the packet (my wife got a vaccination sheet and a little yellow book with dates she received the shots from the clinic in Guangzhou). Technically, according to the instructions, this is all that is needed for K-1 fiancees that had their medical within the past year. Don't be fooled into paying more money to a CS that wants her to go through a medical exam again, just a waste of time and money. K1 doesn't need it.

However, there have been many threads regarding the I-693 form and then need for a civil surgeon to transcribe the vaccination records onto that form regardless. I.e. Some people just sent in the AOS packet and the complete vaccination sheet and still got RFE asking for I-693 filled out by a CS, others were lucky and didn't need it. It's confusing.

If you want to avoid any issues. I suggest going ahead and seeing a CS to have them transcribe the vaccinations taken onto the I-693 and send that off with the AOS packet. It's just a couple of sections on the form that get filled out. No medical exam. They will seal the I693 in an envelope and you send it together with AOS. That's what we did, so far no issues, we've completed biometrics and no RFEs yet. Hoping to have EAD/AP produced soon.

On the other hand, if she still needs to complete her shots, she'll need to do that first and then have the CS complete the I693 for sure. Only way around that is if she's pregnant or some other reason that contraindicates taking the shots.

Also you shouldn't have to pay a whole lot of money for that. We paid $40 to have it done. of course it will depend on the doctor

AWatson, when you are filing for your AOS you will need to include the I693 with your AOS application. If your fiancee did not receive all the neccessary vaccine before her travel to the US she will need to get the require vaccine from the Civil Suregeon. You will need this before you mailed of AOS application also if your fiancee do not need any vaccines all the Civil Suregeon need to do is just fill out the I693 make sure your fiancee have a copy of her immunization card so the doctor can transcrip it. One last thing the Civil Suregeon might try to get your fiancee to do a complete medical if she doen't need it don't get it because it cost a couple hundred dollars to do medical and you might find a Civil Suregeon that might want to charge a arm and a leg to fill out your paperwork. I only paid $25 to get my husband's fill out.
