I am currently self employed and I have a question about filling out the i-134 form.

Which income do I put on my i-134 form? And What income is being looked at if self employed? Do i put my Gross Income or my Adjusted Gross Income?

Correct. Bear in mind they want the amount of income YOU earn, not the gross revenue of the business from your schedule C (if you use schedule C) Personal income is what they are after.

I'm a W2 worker. I downloaded an automatically generated form form my company's website which is specifically used for the purpose of verifying income/employment for immigration purposes.

I can either use this number for the year of 2011, or line 22 on my 1040.

Any preference?

Employed people state their current income which may or may not match any tax return. For salaried, state the annual salary. For full time hourly, multiply the gross hourly by 2080.

Self-Employed people use line 22 from their most recent tax return.
