Hi, I am sending my I-751 this week and just want to make sure I got everything I need, I was bit confused when I read instruction on form which says completed fingerprint card form FD-258 my understanding was that I have to wait for uscis to tell me where I need to go for my fingerprint and there is no form FD-258 on uscis site.

See following item and let me know if I am missing anything that should be included in package.

• Completed and signed I-751

• Check for $545.00

• Copy of the Indian passport and green card of (immigrant spouse).

• Copy of the state driving licenses for both (immigrant spouse and me).

• Original copies of the last two year joint Federal Tax Returns transcript from IRS.

• Copy of the bank statement for 2009 and 2010 showing joint ownership of checking account.

• Summary of Insurance Benefits and Insurance card from Aetna showing joint insurance policy for 2009 and 2010.

• Copy of utility bill from gas company showing joint ownership of account.

• Copy of car insurance policy for 2009 and 2010 showing joint car insurance policy.

• Copy of Airline ticket/conformation page for our trip to India in 2009.

• Copy of statement from family dental showing joint policy.

• Copy of Master cards showing a joint ownership and account.

• Copy of both library cards from town library showing joint ownership.

And i will also include some pictures as well.

Please let me know if I need to add anything else. Thank you R

hi send it f before november 23 201 new fees increase... about your file.when you send it one week later thay send you a letter receipt that you payed and that you have a one year extension you keep that letter for your travals now another two or three weeks later you wil get another letter appointment to take your biometrics with the date and time and address then after all that you wait until thay make a decision. maybe thre or four months.. on your appointment letter you have a new EAC number you can use that to check your EAC number on line good luck.

you do not need a copy of the Indian passport. At this stage, you are a permanent resident in the US and that is all that counts.... to them at least.

jkelly07 is correct - the green card front and back.

Also, disregard all the fingerprinting card stuff. You will receive your biometrics appointment and get that taken care of!

make sure USCIS receives the package prior to Nov 23rd, or else you will have to pay the new fee....

Oh, one more thing - do not send originals... only copies!!!
