I-693 and vaccination question

Timeline

My wife and I have been trying to conceive and we are not sure how to go about the vaccinations and I693. She received the first set of Td, MMR, and Varicella vaccines before her K1 interview 3 months ago. Now preparing AOS paperwork, we need to see a civil surgeon to get the I693 completed. Would the CS mark the remaining shots for a contraindication waiver if she tells says she's planning on getting pregnant?

Also, we have the yellow vaccination history book from China and the 3025 vaccination form. Can we use either one to show the previous vaccinations?

Unlikely. If she IS pregnant then they would do a waiver on the shots that are contra-indicated but in all honesty, if she's planning to get pregnant (but isn't yet) she's better to have all her shots up to date... its good for the baby and for her health.

Take both the DS-3025 and the yellow book.. though the DS-3025 is the one they'll use.
