I've seen her in person three seperate times, what exactly do they want to know?

They want exactly what they ask for. A short explanation of how/when you met in person in the last two years. A simple statement such as "Met on the internet in 2010, met in person in This Country, on This Date."

Do they want to know when we first met or just when we met in person last?

It doesn't matter. If you want, state how you originally met and then say "There have been x amount of visits since, occuring in This Country on These Dates." Don't go into too much detail. They don't want an entire history of your relationship. Just give a date/dates and then back it up with your proof.

It's a matter of establishing you've met in person in the past two years. Pick an instance that you will show evidence of--passport stamps.

Say, "We met so-and-so. I visited on this date." If you can't fit it all, write in the box, "See Supplement page titled "Question 18 Supplement" and describe it there after titling that page the same way.

"While on a trip from Odessa, Ukraine to Prague, CZ on (date) I met Alla at the airport in Prague while waiting in line to buy bus tickets"

Do they want to know when we first met or just when we met in person last?

First time, last time, any time. Take your pick.

The size of the box is indicative of the length of answer they are looking for. I used two sentences.
