Just so there is no confusion: it's actually known as the DS-3025 vaccination worksheet. The vaccination supplement is only issued by approved civil surgeons in the United States.

Hi Sleepybears,

I think you have almost everything covered there is just one thing that I believe might be wrong or you wrote it wrong. The DS-156, DS-156K and maybe DS-230 should be original and you sign them in front of the CO/clerk in the Embassy. One of the DS-156 you took to the doctors but the other original should go to the Embassy. I hope this helps you good luck with the interview.

Peter Miami

Hi Peter,

Hi kwacka,

Yeah thats really a lot of photocopying. But if you see that i'm from Hong Kong (which is a part of China now), I don't think it's such an easy task as the other countries like Canada & the UK. That's why I try to include as much evidence as possible (just in case they want them).

min7734,

yeah I remember that your fiance's interview is the same date as my interview date! So once again, good luck to both of us!!!

Pls give us an update later! :)
