For I-134 Job Verification Letter

Timeline

When preparing my Job verification Letter for i-134, who do we address it to? Can I just have "to whom it may concern"? or the consulate's name and address?

Thank you in advance for all your reponses

The verification of employment letter should be something like: RE: Verification of Employment - [name of the employee]

Then it should have "To whom it may concern: The purpose of this letter is to verify that _____ is an active, full time employee of ______. The position held by _____ is ______ (ex: electrical engineer) with an annual salary of $____.

Dates of employment are _____ to present; and the probability of continued employment is ____(good/bad...)". This is what the verification of employment should contain.

You don't need to address it to the consular officer or something. It should be used to support I-134, together with the bank statement, recent pay stubs and income tax returns for the last 3 years(1040s and W-2s).

The I-134 only requires 1 year of tax returns, unless your embassy/consulate requires more.
