I would like to call people's attention to one of the recommended CSs on the list:

CS Name: Dr. Brian Morris All One Health; The Summer Exchange Building

CS Tel. no: 617-357-4140 (101 Arch Street, Ste. 20, BOSTON, MA 02110)

From our experience, I would not recommend his office for K-1 AOS at all!

Since his office was recommended on this forum, I called and made an appointment for my husband (who came in on a K-1 visa and is in the process of applying for AOS) for just the vaccination report. When my husband arrived, they told him he needed a full medical, despite the fact that he repeatedly told them he did not and pointed them to the portion of the instruction form that clearly states he did not need a medical. They wouldn't see him without it. (They also had no record of his appointment, which was very bizarre since I'd spent a half hour on the phone with the receptionist discussing his situation and confirming that they did vaccination transcription only.) Then they told him the TB test was needed for his vaccination report. This despite the fact that he'd already had all the required tests and chest X-rays done by a civil surgeon in his home country less than 6 months ago.

In any case, he had a positive reaction, which is not surprising since he comes from a country that administers routine BCG innoculations (which produce false positives on TB tests) and he just had a TB test a few months ago, which also increases the already high chances for a false positive. So the doctor's office is now requiring him to have chest X-rays done again, in their office, for an additional $75 fee. They won't accept X-rays done through any other doctor despite the fact that my husband now has health insurance through my company and could have them done for free at his primary care physician's office.

What should have cost $30 has already cost over $200 and required my husband to have unnecessary and excessive medical examinations (including unnecessary x-rays, which are medically contraindicated). It really upsets me that approved civil surgeons seem to share one universal quality: they are all scam artists who have no shame manipulating the fears of others for their own monetary gain.

Dr. William (da Bronx!) Kalafatic was our dishonest civil surgeon.

He should have just done the transcription but demanded the TB test.

In all it cost us $120.

Sorry you when through hard time [$$] with this SC.. Although we are not obligated to retain an specific CS specially if we are not comfortable with his/her approach when insisting/enforcing requirements already obtained/done/processed.

One fact we do need to maintain, the US Health Department does have requirements we have to comply besides the once required by the US Embassy[ies]. As immigrants we are asked for a group of vaccinations prior our interview, in America we May need additional [and I said additional] vaccination not included or required by the USCIS.

Some CS do not accept the Xrates, Blood Work if is not in English and they will do require additional testing... Unfortunately, these guys [CSs] see the opportunity in making more $$ every time they see referrals for AOS..

I am based out of Kentucky and what I did was to call each and every clinic and asked them for their fees and rates. They were all pretty open about it and I only had to pay $25 for my vaccination transcription. :) And for others who required a full physical exam, they only ask for $75 - pretty neat!

Lucky u guys, cuz we paid $575 for my medical!!!!

Its crazy how much money they charge us, at first we paid $475 but they did x-ray for me bcuz they can't tell if my TB is positive or not and the only way to know it is to have the x-ray which is cost another $100!!! Theres 2 assitants and 1 Dr. who examined my TB dot and they can't tell if its positive or not!! I know its not positive bcuz I have my medical before I came here and x-ray too!! Anyways, we spent so much money on that thing which is a ripped off!! I lived here in DFW and the medical place we went is in Irving, I think the name is MOLina Medical Center!! ugh!! :angry:

I used a Dr. Mark Mishkin in Allentown, PA as I was traveling at the time. I found them to be very expensive for the service delivered.

I made an appointment as they only have one person who process immigration medical paperwork and when I arrived, she was not in.

I waited for about one and a half hours for her to arrive. After that things went fairly smooth. I must admit, the Dr was very brief with

his check up.

I had a PPD, Blood check, MMR Shot as I did not have my proof of mine and the 30 sec Dr check and $450 later.. And of course they only take

cash.

There is another dishonest civil surgeon, which is recommended by the USCIS website.

I called him today regarding the I-693 form.

I read some posts here and I am pretty sure that I don't have to get a whole medical examination again. On the phone they told me that they can not just fill out part 2 - Nr. 5 Vaccination information and part 5 the Civil Surgeon Certification on the I-693. All information in Part 2 have to be filled out they told me, therefore I have to take the medical again.

I had my medical examination in January 2010 and I have a brown envelope which states “Do not open this envelope – it contains a copy of your medical examination. Please provide it to USCIS when requested in conjunction with your adjustment of status.”

My guess is just to send this envelope and find a Civil Surgeon who will fill out just Part 2 - Nr.5 vaccination information on the I-693. I also have form DS-3025 proving my vaccation record.

The doctor said that the USCIS will not except it and that I will get just in trouble if I don't do a new medical examination in the U.S.. The cost for everything would be $430! I really think he just wanted to make money and tries to rip me off. I just said thank you.

Can someone give me please some info about it!

Also, do I need to send in the CD which I received after the medical examination in Germany?

It's ignorant when they say they "cannot" do just part II for the form. Go find some other clinic. I did a phone brigade around my area (Louisville, KY) and got really good choices. You do not need to send in the CD. Just bring the current medical exam (valid for one year) - and they just need to transcribe the vaccination part of the form.
