Timeline

I'm helping my cousin fill out her forms for the 129F. On her fiances G-325A (for the Beneficiary) he is not sure about anything for his biological father. Is this going to be a problem? Can he just write Unknown for his fathers first name, DOB, and place of birth?

Also, on the Beneficiary's Birth Certificate, is it required to have both parents names on it?

His birth certificate should at least have the SPACE for both parents. If his father is unknown and isn't listed on the birth cert then it's fine to write "unknown" on the forms. My birth cert lists all that information about my dad so if his dad simply walked out after he was born, he should at least have that info on the birth cert or his mother should have the info. If not, it should be okay to write "unknown".

The same goes for your cousin, if she doesn't have a father listed on her birth cert that's fine, but it should still have a place or have "unknown" on the birth certificate.
