Hi everyone,

I was never given a copy of the DS-3025 at my medical, only the official 'worksheet' which lists which vaccinations I was missing at the time (MMR & TDAP). I have read on here several times that it helps if you can give the civil surgeon transcribing your vaccines, a copy of the DS-3025. I know its inside my packet but as you're not supposed to open it, where would I get one from? Or is the worksheet enough proof for the CS?

That's all my husband had, too. It may vary by CS but he got it done with no problems.

Also, as SOME people have been getting medical RFE's (USCIS has lot their medical etc) I requested an additional copy of the medical "just in case". I only found out about it all once I was here in the states though so I contacted the doctor from here and had them send a copy to my mother who has now sent it on to me (just waiting for the mail :D). So if you're already in the States, give your panel doc a call and ask for a copy of the medical, if they say no, tell them that you're at least required a copy of the immunisations (nicely of course :P) and perhaps they can scan and email it to you, or fax it to someone at home, or mail. Whatever it takes.

**Edit - I just saw in another of your posts that you're still in the UK. It should be VERY easy to get this done while you're in the UK. Much easier than what I'm doing :D
