Yes we used the EZ form....and guess what...it is easy. It doesnt matter which one you use, just make sure it is filled out correctly and he meets the requirements for using the form. We had no issues. I used my 2006-2008 tax transcripts (our AOS was sent in Feb) and used the w-2s from 2006-2009 when I sent it in. I filed my taxes before Mhay's interview and sent her my 1040 from turbotax. they never asked us for 2009, but 2009 is required now. (since april 15 of course  )

Yes it matters which one you use. Only those who qualify can use the the EZ version. If you qualify, you can use either.

I did state that and I bolded the most important area of the instructions that the OP listed.

They must be able answer yes to all three questions, so there is no 1 most important question, which is what you made bold.

Did you elect to us the I-1864 form or did they send it to you? I am starting to look at filling a Affidavid of Support form now. Just wondering..

From what i understood, my hubby chose this form, he read both I-1864 and I-864ez, and since he met the requirements for this one, then he chose to go with this one. From what it looks like, it is a shorter and easier form to fill in.
